## A tiny crack can be easy to miss

A tooth can look clean, white and healthy yet still develop a crack. Some cracks are so fine that they are difficult to notice without a dental examination. A hairline crack may not cause constant pain at first, which is why people sometimes delay seeking advice.

Cracks matter because teeth are exposed to repeated chewing forces every day. Depending on its position and depth, a crack may remain limited, or it may extend deeper into the tooth and compromise its structure. Early assessment gives a dentist the best chance to understand the problem and discuss suitable options.

### What causes a cracked tooth?

Tooth cracks can occur for several reasons, including:

- Biting hard foods or unexpectedly biting on a hard object.
- Teeth grinding or clenching, which can place repeated pressure on the teeth.
- Large fillings or previous dental work that may leave less natural tooth structure.x
- An injury or impact to the mouth.
- Natural wear over time and repeated chewing forces.
- Changes in temperature, which may contribute to stress in some circumstances.

Good oral hygiene is important, but brushing and flossing cannot prevent every crack. A tooth may fracture even when it is free of decay and looks well maintained.

### Signs and symptoms to watch for

Symptoms vary, and some cracked teeth cause little or no discomfort. Possible warning signs include:

- Sharp or intermittent pain when biting or releasing a bite.
- Sensitivity to cold, heat or sweet foods and drinks.
- Pain that is difficult to pinpoint to one particular tooth.
- Discomfort that comes and goes while chewing.
- A visible line, chipped edge or change in the tooth’s appearance.

These symptoms can also have other causes, including decay or gum problems. A dentist needs to examine the tooth to identify the likely cause. Seek urgent dental advice if pain is severe, swelling develops, or you have signs of infection.

### Why the depth and direction of the crack matter

Not every crack means a tooth must be removed. The outlook depends on factors such as the crack’s location, depth, direction, whether it reaches the pulp (the soft tissue inside the tooth), and whether the tooth has separated into distinct pieces. A dentist may use a clinical examination and appropriate tests or imaging to assess the situation.

### How is a cracked tooth treated?

Treatment is individual. The aim is to protect the tooth where possible, relieve symptoms and address any damage to the inner tissue. Options may include:

**1. Monitoring or a conservative restoration**

For selected superficial cracks that do not compromise the tooth, a dentist may recommend monitoring or a suitable restoration. Follow-up is important because the appearance alone does not show whether a crack is stable.

**2. A dental crown**

A crown covers and protects much of the visible tooth. It may be recommended when the tooth can still be restored and needs additional support to reduce the risk of the crack worsening. A crown does not make every crack safe; suitability depends on the diagnosis.

**3. Root canal treatment and a crown**

If the crack has affected the pulp and the tooth is still considered restorable, root canal treatment may be needed to treat the damaged or infected tissue. A crown is often considered afterward to protect the remaining tooth structure. The dentist will explain the likely benefits, limitations and prognosis.

**4. Extraction and replacement options**

If a crack extends too far below the gum line or the tooth has split in a way that cannot be predictably restored, extraction may be recommended. After healing and assessment, replacement options may include a dental implant, bridge or removable prosthesis. Immediate implant placement is not suitable for every case; timing depends on the tooth, bone, gums, infection status and overall treatment plan.

### Can a cracked tooth be saved?

Sometimes, yes. A limited crack may be managed with a protective restoration or crown, with root canal treatment if the pulp is involved. A tooth that has split into separate segments may have a much poorer outlook. Only an in-person dental assessment can determine whether a tooth is restorable and what outcome is realistic.

### What should you do if you suspect a crack?

1. Arrange a dental examination promptly rather than waiting for the pain to become severe.
2. Avoid chewing hard foods on the affected side until it has been assessed.
3. Do not test the tooth repeatedly by biting down hard.
4. Keep the area clean with gentle brushing and flossing.
5. If swelling, fever, facial swelling or severe pain occurs, seek urgent dental care.

## Frequently asked questions

Can a hairline crack be invisible?

Yes. Some cracks are difficult to see and may only be identified through a dental examination and appropriate tests.

Can a clean tooth still crack?

Yes. Cracks can result from pressure, grinding, trauma or other factors even when there is no visible decay.

Does every cracked tooth need a root canal?

No. Root canal treatment is considered when the pulp is affected and the tooth is restorable. Other cracks may need a different approach.

Does every cracked tooth need extraction?

No. Some teeth can be protected with a restoration or crown. Extraction may be recommended if the crack makes the tooth non-restorable.

Can a crown stop a crack from spreading?

A crown can help protect selected restorable teeth, but it cannot guarantee that every crack will stop progressing. The dentist must assess the crack first.
