## Introduction: You are not alone

Bad breath, also called halitosis, is common and can affect confidence in everyday conversations. It may happen occasionally after certain foods or first thing in the morning. If it continues despite brushing, however, it may be a sign that something in the mouth needs attention.

The good news is that persistent bad breath can often be improved once its cause is found. The cause is not always poor brushing. Bacteria on the tongue, gum disease, tooth decay, dry mouth and some medical conditions can all play a role. This guide explains the common causes and practical next steps in plain language.

### Step 1: What is halitosis?

Halitosis is the clinical term for unpleasant breath odour. It can be temporary—for example, after eating garlic or onions, drinking coffee, or waking up in the morning. It can also be persistent. Ongoing bad breath may be related to bacteria breaking down food debris and cells in the mouth, but sometimes the cause comes from outside the mouth.

What this means for you: having bad breath does not automatically mean your teeth are dirty. It is a symptom, not a diagnosis. If it keeps returning, identifying the source is more useful than simply masking the smell with mints or mouthwash.

### Step 2: Understand the common causes

The most common causes inside the mouth include:

- Tongue coating: bacteria, food particles and shed cells can collect on the tongue, especially toward the back, and produce unpleasant-smelling compounds.
- Gum disease: inflamed or infected gums and deeper gum pockets can harbour bacteria. Bleeding gums, swelling or persistent bad breath deserve a dental check.
- Tooth decay or trapped food: cavities, broken fillings, food trapped between teeth and poorly cleaned dental appliances can contribute to odour.
- Dry mouth: saliva helps wash away debris and neutralise acids. Some medicines, dehydration, mouth breathing and certain conditions can reduce saliva.
- Food, drink and tobacco: garlic, onions, coffee and smoking can affect breath. Tobacco also increases oral-health risks.
- Other health conditions: sinus or tonsil problems, acid reflux and some medical conditions may contribute. A dentist or doctor can help decide whether further assessment is appropriate.

Patient tip: notice when the odour is strongest and whether you also have a dry mouth, bleeding gums, tooth pain, tonsil symptoms or reflux. This information can help your clinician assess possible causes.

### Step 3: How a dentist checks the cause

A dental visit usually starts with a conversation about how long the problem has been happening, your oral-care routine, medicines, dry-mouth symptoms and any other concerns. The dentist may examine your teeth, gums, tongue and oral tissues, looking for plaque, gum inflammation, decay, food traps or other possible sources.

Depending on what is found, the dentist may recommend professional cleaning, gum treatment, care for tooth decay, or changes to your home routine. If the mouth does not appear to explain persistent symptoms, you may be advised to see a medical professional for further assessment.

What this means for you: an examination aims to find the cause rather than judge you. Be open about your symptoms so the care team can make the most helpful recommendation.

### Step 4: Treatment depends on the cause

There is no single treatment that works for every case. Your dentist may recommend one or more of the following:

- Professional cleaning to remove plaque and hardened tartar that brushing cannot remove.
- Gum treatment if gingivitis or periodontitis is present.
- Treatment for tooth decay, defective fillings or other areas where food and bacteria collect.
- A daily tongue-cleaning routine to reduce coating without scraping aggressively.
- Help managing dry mouth, including hydration advice and reviewing possible contributing factors with a healthcare professional.
- A medical referral when symptoms suggest a cause such as sinus, tonsil, reflux or another health condition.

Mouthwash may help some people, but it should not replace brushing, interdental cleaning or treatment of gum disease and decay. If a product causes irritation or symptoms continue, ask your dentist for advice.

*Visual guide: common causes, tongue cleaning and practical steps that may help improve breath freshness.*

### Step 5: Your daily routine for fresher breath

1. Brush twice daily for two minutes with fluoride toothpaste.
2. Clean between your teeth daily with floss or interdental brushes.
3. Gently clean your tongue once a day with a toothbrush or tongue scraper.
4. Drink water regularly, especially if your mouth feels dry.
5. Clean dentures, retainers and other removable appliances as instructed.
6. Avoid smoking and seek support if you want help stopping.
7. Attend regular dental check-ups and professional cleaning when recommended.

### When should you book an appointment?

Arrange a dental appointment if bad breath continues despite a consistent oral-care routine, or if you have bleeding or swollen gums, toothache, loose teeth, persistent bad taste or visible tooth problems. Seek prompt care for facial swelling, fever or severe dental pain. If the dental examination does not identify an oral cause, your dentist may suggest medical assessment.

### How TAG Dental Clinic can help

If you are concerned about persistent bad breath, TAG Dental Clinic can assess your teeth, gums and tongue, discuss your symptoms and explain suitable next steps. Treatment is based on the findings of your examination, because the most effective way to improve breath freshness is to address the cause.

## Frequently asked questions

Is bad breath always caused by poor oral hygiene?

No. Tongue coating and gum disease are common oral causes, but dry mouth, foods, tobacco and some medical conditions can also contribute.

Can cleaning my tongue help?

Yes. Gentle daily tongue cleaning can reduce coating and odour for many people, especially when combined with brushing and cleaning between the teeth.

Does mouthwash cure halitosis?

Not always. It may temporarily reduce odour, but persistent bad breath often requires identifying and treating the underlying cause.

Why do I have bad breath even after brushing?

Odour may come from the tongue, gum pockets, tooth decay, dry mouth or a non-dental cause. A dental assessment can help narrow it down.

Can a dentist help if the cause is medical?

A dentist can check for oral causes and recommend medical assessment when symptoms suggest the source may be outside the mouth.
